REFERRAL PROCESS

Please ask your primary care provider to send a referral to Dr. Purcell
To schedule an appointment with Dr. Purcell, our office requires a referral from your primary healthcare provider, such as your family doctor, nurse practitioner, or other provider.


* Please Note:
• We cannot schedule appointments without a referral.
• Relevant imaging, such as a CT scan of your sinuses, can be arranged by your primary healthcare provider before your appointment as needed,
• Sinus x-rays will not be accepted

Referrals received by the office are reviewed by Dr. Purcell and scheduled accordingly. Patients should expect a phone call with available appointment times.

At your first appointment, you will be asked to complete a patient information survey. 

This survey will ask you questions about yourself and the symptoms you are experiencing. 
Survey information will be used to track your progress, and conduct research to improve patient outcomes and experiences.
